IRB Application

SUNY Cortland's Institutional Review Board (IRB) uses a primary review procedure that begins with submitting information and required documents to the IRB. To ensure your IRB application is reviewed in a timely manner, submit applications, consent/assent forms, and all supporting materials as an email attachment to irb@cortland.edu. If the IRB cannot process your application because documents are missing, the application is not complete or has been inadequately completed you will receive notification.

Original signature-bearing documents must be sent by U.S. Mail or intercampus mail, addressed to Institutional Review Board, PO Box 2000, Miller, Room 303, Cortland, NY 13045. Examples of signature-bearing documents include: Appendix A (signature page); institutional permission letters; Memorandum of Understanding (MOU), etc. Investigators conducting research off campus can submit Appendix A by fax, and can ask outside agencies to send their permission letters, MOUs, etc by fax as well. This policy is consistent with current federal regulations concerning the submission of signature-bearing documents.

When all documents have been received, exempt and expedited applications are forwarded to the Chair and/or the primary reviewer. Primary review takes approximately two weeks. Within this period the primary reviewer will review the application and either make requests for amendments and/or revision by the PI or approve the application.

If you believe that you have a full board review application, please contact the IRB Chair to discuss the classification of the protocol. Confirmed full board review applications are forwarded to the IRB Chair for placement on the agenda. Full board meetings and submission dates are listed on the Institutional Review Board's main web page .

A full board protocol must be submitted at least two weeks before the scheduled meeting.


Applications and Instructions

Before you submit an IRB application, make sure you are CITI certified. First, download and read these enrollment Instructions. You will not be approved to conduct any research at SUNY Cortland until you have the certification. When you are ready to apply, download the appropriate application for the research (see OHRP Decision Charts or contact the IRB to confirm the level of IRB review appropriate to your research).


Surveys

If you intend on using surveys in your protocol, please refrain from using Google Forms or Google Docs. Use the campus supported Form Stack or Select Survey to create your survey. If you have any questions on an alternative survey creator you plan to use, please contact Joshua Peluso.
